Use this form to request a midcourse or final exam. Remember that to request a final exam, all lessons must be submitted and graded. You can check to see if all your lessons are graded by visiting GradeCheck.

Note: You cannot request a retake exam with this form. Please call Independent Study at 1-800-914-8931 or 1-801-378-2868 to request a retake exam.

Students taking exams at BYU Independent Study must call or have this form in our office twenty-four hours prior to taking the exam. Testing hours are 8:00 am - 4:50 pm, Monday - Friday.

Note: If you are taking an exam at the Independent Study office or the BYU Salt Lake Center, you are required to adhere to BYU dress standards. Clothing is inappropriate when it is sleeveless, revealing, strapless, backless, or form fitting. Dresses, skirts, and shorts must be knee length or longer. Hairstyles should be clean and neat, avoiding extremes in styles and colors. Excessive ear piercing (more than two per ear) and all other body piercing are not acceptable.
